ITB TRD01 HVAC 260832 B4682 Work Bay Exhaust System Maintenance

The Awarding Authority seeks a Limited User Contract to award one (1) HVAC contractor to perform the maintenance of eight exhaust fan units and all subsequent items in the included specifications 260832 B4682 Work Bay Exhaust System Maintenance at 4682 Robert Rooney Rd, Camp Edwards, MA 02644.

The estimated contract value is **$35,000.00**. Project is subject to Department's availability of funds and appropriation.

Proposal will include all labor, tools, and equipment necessary to complete the following Scope of Work:

Work will include but not be limited to: Fulfill all maintenance items for the eight exhaust units in the work bays in building 4682 including: replace fan belts, clean fan blades & shroud, grease all fittings, lubricate bearing (if applicable), and inspect all components of the exhaust fans for correct voltage, damage or corrosion. Report all damages found to the PM.

All inspection/maintenance/certifications as identified in the equipment's manufacturers O&M manuals and service bulletins, and standard practices for the trade, and/or required by MA Codes which are not specified or described herein shall be deemed part of this specification.

**Bid Submission:** Participating vendors shall submit a completed *General Bid Form* with a lump sum cost for the project scope of work. A 5% bid bond must accompany a completed general bid form and be uploaded with the bid if over $50k.

Only electronic quotes submitted via COMMBUYS will be accepted in response to this ITB. Responses must be sent via the "Create Quote" functionality in COMMBUYS.

**Required Forms:** Contractors submitting a bid must include an *Authorized Signatory Form*, *Non-Collusion Form*, and *MA Tax Compliance Form*.

**Wages:** Prevailing wages will be required for this project. Certified payroll is required prior to progress or final payment.

**Bonding and Insurance:** A Payment bond in the amount of 50% of the project cost will be required from the awarded contractor for projects exceeding $25K. Payment Bonds and Performance Bonds in the amount of 100% of the project cost will be required for bids over $150K. The awarded contractor will produce Liability Insurance information naming the Commonwealth as additionally insured prior to work commencing.

**Evaluation:** A lump sum bid will be evaluated using M.G.L. c. 149 44 A-J by the Awarding Authority. The Contract will be awarded to the lowest responsible and eligible Bidder as determined by the Awarding Authority.
