Medical Equipment and Supplies for Lincoln Fire & Rescue

It is the intent of the City of Lincoln, Lincoln Fire & Rescue Department to contract medical equipment and supplies from more than one (1) vendor.

Orders will be placed on an as-needed basis with most of the time being weekly.

Pricing shall be FOB destination.

Deliveries shall be made to:

- Between the hours of 8:00 a.m. and 4:00 p.m., Monday through Friday.

Contract Administrator is Jamie Pospisil.

If vendor bids an equivalent, they shall attach the technical specifications in the Vendors Response Attachment section of the E-bid.

Each bid line includes a line-item attributes for vendors to complete requiring the Manufacturer and Model number of the item you are bidding along with packaging information.

I acknowledge that the quantities listed for each line item are an estimated yearly amount. The City does not guarantee any dollar amount or order quantities for the term of the contract.

Bidder shall submit bid documents and all supporting material via e-bid.

Bidders must be registered on the Owner’s Ebid site to submit a response to any solicitation.

**Required:** Complete and upload the *Foreign Adversary Contracting Prohibition Act Form* in the Attachments tab in Ebid. Complete and upload the *U.S. Citizenship Attestation Form* from the Attachment tab.

The award determination will be made in accordance with section 13. BID EVALUATION AND AWARD of the “Instructions to Bidders” attachment and also with consideration given to the following factors:

• Pricing provided by bidder
• Bidder’s ability to meet the requirements as outlined herein, in e-bid Line-Item Descriptions and Notes and any related bid documents.
• Meeting specifications and or bid lines as requested.

Contract term is for one (1) year with the option to renew it for three (3) additional one (1) year terms upon mutual consent of both parties. With the contract term beginning upon its execution.

Bid security, as a guarantee of good faith, in the form of a certified check, cashier's check, or bid bond, may be required to be submitted with the solicitation, as indicated on the solicitation.

No response shall be modified or withdrawn for a period of ninety (90) calendar days after closing date and time.
