FY 27 IVHM Elevator Repairs

The Vendor shall provide all labor and materials necessary for the repair to elevator, preventative maintenance, and annual inspection for the Illinois Veterans Home Manteno (Facility) located at One Veterans Drive Manteno, IL60950.

The Vendor shall provide all labor and materials necessary for elevator repairs, two (2) preventative maintenances, and an annual inspection.

Elevator Repairs

• Replace two (2) existing out-of-service freight elevator door pancake motors
• Replace associated braking rectifier assembly.

• Door Motors. Model: Courion LP25 Door Motor. Part Number: 90-854000
• Braking Rectifier. Model: Courion Braking Rectifier Assembly. Part Number: 01-100800

Preventative Maintenance

Provide semi-annual preventative maintenance visits to include inspection, testing, adjustment, cleaning, and required lubrication for the

• Control Systems
• Power unit and associated systems
• Hydraulic system components and accessories
• Hoistway equipment
• Pit equipment
• Door equipment
• Signals and accessories
• Rail and guide systems

Annual Fire Marshall Inspection

• Provide necessary services, labor, and/or materials to complete required annual elevator inspection in accordance with the requirements of the Illinois Office of the State Fire Marshal Elevator Safety Division
• Utilize inspector approved by the Illinois Office of the State Fire Marshal
• Perform all testing required by the State Fire Marshall Inspection checklist
• Provide completed copy of inspection report and all associated documentation to the Chief Engineers’ Office
• Provide necessary documentation for issuance of Elevator Certificate of Operation.

General Provisions

• Vendor shall verify all parts utilized in services rendered at the facility are compatible with existing equipment and elevator devices.
• All work and inspections to meet requirements of the Illinois Elevator Safety and Regulation Act 225 ILCS 312.
• Vendor will NOT complete any additional repairs or services not stated within the contract.
• Vendor to provide a minimum of a 90-day warranty to include repair or replacement of defective materials and correction of workmanship deficiencies.
• NO sub-contracting of services will be accepted by the facility

Vendor will coordinate/schedule all work with the Chief Engineers Office. All work shall be completed during normal business hours Monday-Friday 08:00 AM – 04:30 PM excluding State and Federal Holidays, unless otherwise preapproved by the Facility Chief Engineer or their designee.

The contract will have an initial term commencing upon execution and ending on **June 30, 2027**.

Vendor shall provide: (a) General Commercial Liability insurance in the amount of **$1,000,000** per occurrence (Combined Single Limit Bodily Injury and Property Damage) and **$2,000,000** Annual Aggregate; (b) Auto Liability, including Hired Auto and Non-owned Auto (Combined Single Limit Bodily Injury and Property Damage), in amount of **$1,000,000** per occurrence; and (c) Worker's Compensation Insurance in the amount required by law.

The *IPG Active Registered Vendor Disclosure* and *Standard Illinois Certifications* are included with the solicitation.
