REBID- DHS SP - Kiley - Replace Water Valve Distribution System- FY27

Provide Materials, Labor and Equipment to Replace Two Underground Water Distribution System Isolation Valves at the Ann Kiley Center, 1401 Dugdale Rd, Waukegan, IL 60085, for the Illinois Department of Human Services.

• Excavate, remove and dispose of two existing 8" isolation valves, at sites indicated by Facility.
• Install new valves in accordance with local codes, NFPA standards and Illinois Plumbing Code procedures that apply.
• Satisfactorily test operate valves and provide valve key.
• Restore any disturbed landscaping or pavement resulting from valve replacements.
• Waste generated by valve replacement must be removed by Contractor.

Notes

All contractors are **required** to make site visit prior to submitting bid.

Awarded vendor must be licensed Plumber by the State of Illinois, bonded and insured.

**Mandatory self-performance:** The Prime Vendor shall perform on the site, and with its own organization, work equivalent to at least 51% of the total amount of work to be performed under this Contract.

Vendor must check "YES" checkbox to indicate compliance with the Illinois Prevailing Wage Act for bid to be considered responsive.
