You are invited to review and respond to this Invitation for Bid (IFB) for fire life safety maintenance services being offered by the Department of General Services, Facilities Management Division (FMD).

FMD is procuring services for Fire Life Safety (FLS) System maintenance and repairs on state property.

Services will be performed at the following location:

50 D Street
Santa Rosa, CA 95404

The Contractor shall provide all labor, equipment, and materials required to provide fire life safety maintenance services for the Fire Life Safety (FLS) System equipment identified in Exhibit A, Attachment 1, Equipment List of Attachment 9, Sample Contract.

The anticipated term of the contract will be **36 Months**.

The Bidder's rates will stay in effect for the entire contract term.

The Bidder's required services are provided in Attachment 9, Sample Agreement. The winning Bidder will be required to adhere to all Agreement terms as indicated in Attachment 9, Sample Agreement.

This contract may be subject to Prevailing Wage. Contractor and Sub-contractor(s) must be registered with DIR, if required by law to do so, in order to submit a bid.

Contractor shall be responsible for supplying miscellaneous materials (i.e., grease, lubrications, gloves, etc.) incidental to service, including safety materials needed or required to perform service, at no additional charge to the State.

**Submission:** The Bidder will deliver their sealed bid in such a way that the outside is clearly labeled. Hand delivered bids shall be placed in the DGS/OBAS Bid/Proposal Drop Box located in the lobby at 707 3rd Street, West Sacramento, CA 95605. Facsimile (FAX) and electronic submissions will not be considered.

**Minimum requirements:** Bidders that do not meet all minimum requirements will be rejected.

• C-16 Fire Protection license issued by the California Department of Consumer Affairs, Contractors State License Board
• Current proof of insurance (*Attachment 7, Proof of Insurance*) for General Liability, Commercial Automobile Insurance and Worker's Compensation
• Three (3) or more references (*Attachment 5, Bidder References*), **required**
• **3%** DVBE participation, documented on *Attachment 3, Bidder Declaration (GSPD-05-105)*

**Award:** Contract award, if made, will be awarded to the Bidder whose bid has the lowest responsive responsible submission.

The non-mandatory walk-through is open to Bidders, who will need to present identification and sign in at each facility.

Upon award of the contract, the winning Bidder must provide a COI within ten (10) working days from award of contract.
