The purpose of this Request for Bid is to solicit competitive bids from qualified contractors for Indian Hills Locker Room Locker Repair & Replacement for Canyons School District (CSD). It is anticipated that this RFB may result in a contract awarded to a qualified contractor for commodities as defined further in the solicitation.

Canyons District is soliciting bids to partially replace and repair the lockers in the boys locker room that were damaged due to vandalism over several years.

Scope

• Replace lockers in the clouded area to match existing. Page A422 for reference.
• Salvage parts to repair the remaining lockers in the space.
• Add 694 combo locks to all lockers in the space to prevent future damage.

Bids will be opened electronically within five (5) business days of the closing of the solicitation.

The bidding Supplier must be properly licensed to do business in the State of Utah.

Each bidder shall submit only one bid. Alternate or multiple bids from a single supplier are prohibited. On any item where multiple bids are submitted, that item may be deemed non-responsive. Bids will be evaluated for content, price, specification requirements, and availability.

CSD requires all bidders to submit a list of ALL first-tier subcontractors on the *Subcontractors List* form. A **mandatory** pre-bid conference applies.
